Comprehensive aesthetic rehabilitation after orthodontic treatment and gummy smile correction

Complaints:

Crooked teeth, loss of several teeth, lack of confidence in the smile, joint clicking, and bleeding gums.

Diagnosis:

Distal deep bite, crowding of the anterior teeth, narrow dental arches, pronounced gummy smile, and the presence of old leaking restorations.

Treatment:

A comprehensive diagnostic examination was performed, including a photographic protocol and digital 3Shape scanning. The patient was referred for orthodontic treatment to correct the bite and tooth position before further implant placement and aesthetic rehabilitation. Over the course of 1.5 years, the dental arches were aligned, the midlines were corrected, and the position and inclination of the teeth were adjusted. After completion of the orthodontic stage, a repeated diagnostic assessment and digital smile design were carried out. Laser gum contouring was performed using a Picasso laser according to an individual gingival guide. A Wax-Up was created, 3D models were fabricated, and a Mock-Up stage was performed to evaluate the future tooth shape and approve the planned result. Under the control of a Seiler operating microscope, minimally invasive tooth preparation within the enamel was performed. After that, metal-free E-max ceramic restorations in the lightest shade were fabricated and permanently bonded.

In this case, the patient was concerned about crooked teeth, missing teeth, lack of confidence in her smile, clicking in the temporomandibular joints, and bleeding gums.

Achieving the desired result required comprehensive treatment involving specialists from different dental fields.

During the diagnostic examination, a distal deep bite, crowding of the anterior teeth, narrow dental arches, a pronounced gummy smile, and old defective restorations were identified.

All of these factors affected both the aesthetics of the smile and the function of the dentofacial system.

Treatment began with comprehensive diagnostics, a photographic protocol, and digital 3Shape scanning.

For one and a half years, orthodontic treatment was carried out, allowing the dental arches to be aligned, tooth positions to be corrected, and the teeth to be prepared for further rehabilitation.

After the orthodontic stage was completed, digital smile design, laser gum contour correction, and a Mock-Up were performed to evaluate the future result.

Then, under the control of an operating microscope, minimally invasive tooth preparation was performed and metal-free E-max ceramic veneers were placed.

Result:

A harmonious smile was created, proper tooth proportions were restored, and the pronounced gummy smile was eliminated.

The smile became more aesthetic, brighter, and more natural, while the comprehensive approach resulted not only in visual improvements but also in enhanced dentofacial function.
